Ans:

We have seen and successfully treated so many couples married for several years without having sexual intercourse. This condition is called unconsummated marriage. The couple with longest duration I have seen is of 22 years.  Non consummation of marriage can occur due to several reasons. It is due to problems related to either husband, wife or both. Male problems such as erectile dysfunction, premature ejaculation, Sexual desire disorder, dyspareunia, phimosis, peyronies disease and female problems such as absence of sexual desire, sexual arousal disorder, dyspareunia, sexual aversion disorder, vaginismus etc can cause non consummation of marriage. Other psychological problems such as adjustment problems, marital conflicts or dislike towards the partner also can lead to such conditions. How ever nothing to worry about it. With modern investigative and treatment modalities  we can identify the cause and treat it successfully.

Vaginismus can be successfully treated with a three weeks inpatient psyco-sexual therapy.

i have a problem that my penis is not attining the steady state for long time it will shorten when the hand is taken from it with in second,both in time of mastruburation and the time of sexual contact.this problem starts only before 2 months.am little tensed because of this problem.how can i over come this.am sure that am mentaly perfect.is this a phisical problem and wats the solution,there is any medicine for this ,please help me.

Hi Doctor, Unconsummated marriage is my problem.It s been 6 years since we married.I am 38 years old and my wife 34.Working in middle east.,we both are professionals.I don t think I have physically some sexual issues.The problem is I don t feel to engage in a sexual intercourse with my wife..lacking a sexual attraction to her..In the beginning,we tried to have intercourse and she felt some pain and I had withdrown from penetration..then gradually I felt a kind of less sexual attraction (or no sexual attraction) Then we never made a try (some 3-4 times maximum)..Except this matter,we are in good relation,good understanding,caring and love..In my teen age,I had a chance of engaging an intercourse with an elder women (situational incident)and because of my tension it didnt work and ended up with out anythng and I left..Later on I myself had an impression that It was probably because of mental tension,not physical issue..and an intimate partner can resolve those mental issues etc..i got an intimate partner,but still I am not able to overcome it..I think this tim e her pain issues have taken me back..I am not sure (and I dont think it is an exceptional pain,but a normal one)..Our s was a love cum arranged marriage..sometimes I feel ,may be that pressure also had played some role on this issue..but now we are ok. All our family members are expecting a kid..We are very diappointed of not having a kid also..I am sorry,if i made a big story and condfusing you...but please if u can guide me,I would be more than happy...

Q:

Dear Doctor, I am 39 years old. Married with 2 kids, having a very good sexual life. But recently started observing that strong erection happens initially during foreplay and after when penetration starts sometimes the penis loses its firmness and becomes sloppy. We both indulge in foreplay first and try to satisfy her first by foreplay then go for the love making, but now due to this issue we are starting to get worried. Ans:

Usually the curvature of penis occurs due to the development of plaques inside the penis. It will lead to curvature and pain during erections. You may be able to feel a hard area on the penis at the site of the curvature or on the opposite side. This condition is known as Peyronie's disease. If it is at the early stage it can be treated with medicines. Otherwise you may need a surgery to remove the plaques. It all depends on the extent and severity of the disease. In our hospital we do scanning and other relevant investigations before planning any treatment. Depending on the results we plan the treatment. In certain cases if the plaque is large and situated in the corpus cavernosa or corpus spongiosum there is a risk of losing erectile ability as a complication of the surgery. One can predict any such risk after appropriate evaluation. Before planning any surgery this has to be considered and sufficient precaution should be taken to solve if any such emergency occurs. You can consult an andrologist for this treatment.

If any of the partners is not satisfied we can assume that there is a problem. Inability to control the ejaculation may be due to several reasons. Sometimes this may be a habit. In some other cases such problems may be due to infection, inflammation, hypersensitivity, or any other factors. First we have to find out the cause. We are married for 14 years and my wife have severe pain during sexual intercourse. Due to this we are mostly satisfied with oral sex. There are different causes for premature ejaculation. This may be due to infection, inflammation, hypersensitivity, habituation or any other factors. First the etiology should be identified. Treatment may vary from medication to sex therapy or a combination of both based on the etiological factors. To decide the treatment you need to undergo a clinical evaluation including physical examination and investigations. With out that it cannot be treated. Definitely there is a cure for this problem.
