We were married in the year 2003. we dont have kids. We have undergone tests and came to know that my husband is having azoospermia and he is not having any other problem regarding erection or in doing sexual intercourse but the problem is that he is not having any interest in sexual life.He is taking some medicines for depression for the last ten years. We are undergoing some Ayurvedic treatment for the last one year under my compulsion. So its my humble request whether we can have a healthy happy life. Is there any remedy to rectify his lack of interest in doing sexual intercourse.

Ans:

Azoospermia is a condition with complete absence of sperm in the semen.  It can be due to various reasons. One of the common causes is an obstructive azoospermia which means an obstruction in the reproductive tract or passage of sperms. Matured sperms are collected in the epididymis and carried through the vas deference to the ejaculatory duct. Any obstruction in this path can cause azoospermia. In a few men vas deference is absent by birth. This is another cause of obstructive azoospermia. Absence of spermatogenesis (Spermatogenesis is the process that creates sperm from initially undifferentiated germ cells) or maturation arrest is another cause.  Maturation arrest is the cessation of the development of spermatogonia to sperm cell.

You have to undergo complete evaluation to find out the cause. Evaluation includes physical examination, semen analysis and sonological evaluation. Only after detecting the actual cause, an opinion about treatment can be given. We have all the facilities to evaluate such conditions.

There are different causes for premature ejaculation. This may be due to infection, inflammation, hypersensitivity, habituation or any other factors. First the etiology should be identified. Treatment may vary from medication to sex therapy or a combination of both based on the etiological factors. To decide the treatment you need to undergo a clinical evaluation including physical examination and investigations. With out that it cannot be treated. Definitely there is a cure for this problem.

Erectile dysfunction is treatable. Treatment varies from sex therapy, medicines or surgery. It can be planned only after appropriate investigations and physical examination. Without  this we are unable to give an opinion.

Ans:

Homosexuality is of two types. The first is ego syntonic in which the individual enjoy and it doesn cause any mental stress to the person. Second type is ego dystonic in which the individual feels mental agony and distress about his own homosexual behaviour and hence wants to change it. No treatment is indicated for the first group. Ego dystonic homosexuality can be treated if the individual has a genuine motivation to change his behaviour.

Treatment is indicated only if the person has any discomfort or problem due to his own homosexual tendency or behaviour. The desire to change the orientation should not be due to others (family, friends, religion etc) pressure. Treatment will be successful only if the person has a very genuine and consistent motivation to change his behavior. There is no medicine to change the sexual orientation. Treatment is psychological using techniques of sex therapy based on cognitive behavioural principles. Usually we don take any homosexuals for treatment. But we have treated a few men with genuine motivation and they recovered.

Q:

I have both ED and PE. I am 48 yrs old. As suggetsed by a doctor, I have had Sifasi IU 5000 once a week for two months; Argin 5G Sachet twice a day for two months; Kutub 30 mg for one month; UbiCar & Lvate for one month. But there is no any appreciable difference in both ED and PE. We need to have two more children in our lives. I need to have healthy sexual life. what do u suggest for me?

Q:

I am almost 50 yr old man and do have ED and Premature ejaculation. I would like to do the penile implant (Rigid). My doubt is, once implanted, whether the penis will be always in the erected stage only in which case it is diificult to bend it and conceal it while wearing especially pants or is it that when we need an erection then only it will be erect? and, when it is said, the implant is lifelong, is it that it does not require any regular check-up? And, at last, how much it will cost a patient with all the required investigations or just the implant-- I am not talking about an exact figure, but at least the amount may range say from Rs. 15000 to 25,000 or say 40,000 to 50,000 or so including the hospitalisation charges. Ans:

From our experience of treating thousands of patients with sexual problems from Kerala and outside it is found that 99% of men complaints of small penis got apprehension and doubt about the size of their sexual organ. It is only a concern, and not a fact. The size of flaccid penis is immaterial and it will keep on changing based on the atmospheric and body temperature. Most men worry about the size of the flaccid penis. The only published research study conducted in India on penile size is from Dr. Promodus Institute of Sexual & Marital Health which is published in the International Journal of Impotence Research (2007, 19,558-563). The mean erected length is found to be 13.01 cm with a SD of 1.62 cm.  The summary of this study is available at the Institute’s website (www.drpromodusinstitute.in) and the full paper is available in the internet at the journal’s site.

You need not worry about the penile size. You may consult a doctor who can examine your body and alleviate your apprehensions.

Dear doctor, After my marriage my hus knew that i don t knw anything abt intercourse. Ans:

We have seen and successfully treated so many couples married for several years without having sexual intercourse. This condition is called unconsummated marriage. The couple with longest duration I have seen is of 22 years.  Non consummation of marriage can occur due to several reasons. It is due to problems related to either husband, wife or both. Male problems such as erectile dysfunction, premature ejaculation, Sexual desire disorder, dyspareunia, phimosis, peyronies disease and female problems such as absence of sexual desire, sexual arousal disorder, dyspareunia, sexual aversion disorder, vaginismus etc can cause non consummation of marriage. Other psychological problems such as adjustment problems, marital conflicts or dislike towards the partner also can lead to such conditions. How ever nothing to worry about it.
