Chronic diabetes mellitus is one of the most common causes of erectile dysfunction. 21 years is a pretty long period. It is quite possible that some diabetogenic changes might have occurred in the corpus cavernosum inside the penis which can lead to an erectile problem. It has to be evaluated first and the treatment should be planned based upon the findings. In certain cases medicines may be effective. In such instances it can be treated with a combination of medication. In other cases where medicines are not effective surgery is the only answer and it the best as well as a life long remedy. In your case it can be planned only after the evaluation.

It is known as male orgasmic dysfunction. You are not able to experience the pleasure during ejaculation. Often it is psychological problem. Usually men with depression are not able to achieve an orgasm. Problems like depressive cognition, negative attitude, mental stress, obsessive traits etc can impair the orgasmic response. Normally, it is a difficult condition to treat. How ever there is treatment for the same.

Dear Doctor, I have been married just four months back. In these four months my husband approached me only five to six times for sexual intercourse and we could do only four times sucessfully in these four months. Also his penis was not 100 percent erect twice, so he lost interest during those two times when were about to have intercourse. And the most disappointing part is he does not have any interest in this subject, I will be waiting for him to come home from office entire day and when he comes in the night he eats dinner and just sleeps off.
