Q:

Im 37 year old.i have 2 kids.Now im suffering with varicocele problem.I consult with a Eurologist. He is advising me to do a surgery.My question is ,if i do a surgery does it effect my sexual ability?Any chance to come the same problem later ?. My scan report details. Right side at rest 1.8 mm with Valsalva 4mm(suspine posture).Left side at rest 4.4mm with Valsalva 3.9mm. And my sperm count is 15 million/ml. Waiting for your advise. Warm Regards Siby James.

Ans:

Varicocele causes progressive and duration dependent damage to the testis. As a result sperm motility will be reduced first followed by count. Over the years testis become atrophied and the production of male hormone (testosterone) will reduce. It will cause loss of sexual desire and erection. In other words you may develop an early andropause.

If there is a varicocele in an unmarried man one must undergo microsurgical varicocelectomy. This is the latest scientific view and standard teaching on this matter. How ever in most of the places varicocele is usually operated using laparoscopic surgery or conventional surgery.

The most common complications of conventional varicocele operations are hydrocele, varicocele recurrence, and injury to testicular artery, spermatic cords etc. A person having varicocele should undergo microsurgical varicocelectomy before going for IUI, IVF or ICSI to improve the sperm quality. It will reduce or nullify the adverse effect of ROS (reactive oxygen species) on the sperms. Presence of varicocele will reduce the percentage of success when going for artificial reproduction techniques.

In microsurgery the use of the operating microscope allows for reliable identification of spermatic cord, lymphatics, internal spermatic veins and venous collaterals, and the testicular artery or other arteries. So that chances for these complications are almost reduced to less than 1 percent. Repair of varicocele results in greater improvement in sperm motility and count.

Microsurgical varicocelectomy provides a safe and effective approach to varicocele repair with preservation of testicular function, improvements in semen quality, and improvements in pregnancy rates. So in your case the best solution is bilateral microsurgical varicocelectomy.

It may take few months to resume there normal menstrual cycle after delivery. It becomes delayed when they are breast feeding. There is nothing abnormal in it. There is a lot of hormonal changes during delivery and lactation. It will become normal gradually. Some times the hormonal changes can affect the sexual desire. More than that the arrival of a precious baby can change her mind as she has to spent more time to look after.  Wait for a few more months. If her condition doesn improve discuss the matter with her. If necessary you can have a consultation after some time.

Q:

Sir, when my penis is erect it is curved downwards ..is it normal?can it be corrected? I am afraid to marry due to this problem and i feel my left testicle is smaller than right..i have no problem during ejaculation..is it normal?

Ans:

Usually the curvature of penis occurs due to the development of plaques inside the penis. It will lead to curvature and pain during erections. You may be able to feel a hard area on the penis at the site of the curvature or on the opposite side. This condition is known as Peyronies disease. If it is at the early stage it can be treated with medicines. Otherwise you may need a surgery to remove the plaques. It all depends on the extent and severity of the disease. In our hospital we do scanning and other relevant investigations before planning any treatment. Depending on the results we plan the treatment. In certain cases if the plaque is large and situated in the corpus cavernosa or corpus spongiosum there is a risk of losing erectile ability as a complication of the surgery. One can predict any such risk after appropriate evaluation. Before planning any surgery this has to be considered and sufficient precaution should be taken to solve if any such emergency occurs. You can consult an andrologist for this treatment.

Q:

Am not sure about my sex orientation as Gay or Bi-Sexual. Not had sex with a girl yet but had sex with many boys. So there an treatment to change? Am i able to marry a girl?

Ans:

Homosexuality is of two types. The first is ego syntonic in which the individual enjoy and it doesn cause any mental stress to the person. Second type is ego dystonic in which the individual feels mental agony and distress about his own homosexual behaviour and hence wants to change it. No treatment is indicated for the first group. Ego dystonic homosexuality can be treated if the individual has a genuine motivation to change his behaviour.

Treatment is indicated only if the person has any discomfort or problem due to his own homosexual tendency or behaviour. The desire to change the orientation should not be due to others (family, friends, religion etc) pressure. Treatment will be successful only if the person has a very genuine and consistent motivation to change his behavior. There is no medicine to change the sexual orientation. Treatment is psychological using techniques of sex therapy based on cognitive behavioural principles. Usually we don take any homosexuals for treatment. But we have treated a few men with genuine motivation and they recovered.
